Supportive Home & Community Living
A safe, supportive place to call home where adults can grow toward greater independence with the daily support they need, including assistance with daily living activities, meal preparation, laundry, transportation, and community participation.
Life-Skills & Transition Support
Coaching and guidance to build everyday skills and navigate life's transitions — from school to adulthood — at each person's own pace.
Community Connection
Opportunities to take part in favorite activities, build friendships, and be a welcomed, active part of the community.
We're now accepting referrals and inquiries, with services beginning October 5, 2026. Additional details on eligibility, funding options, and availability will be shared as our programs launch.
